Manage a laboratory safety incident

Manage a laboratory safety incident through life protection, hazard isolation, emergency coordination, exposure assessment, evidence preservation, and authorized recovery.

Follow local emergency procedures and trained responders; do not improvise technical cleanup in an unknown hazard.

When to use

  • Use for chemical, biological, radiological, electrical, pressure, fire, cryogenic, mechanical, sharps, exposure, containment, ventilation, or facility incidents.
  • Use routine spill or equipment procedures only when the hazard is known, within trained limits, and no injury, exposure, escalation, or emergency condition exists.

Preconditions

  • Know alarms, evacuation routes, emergency numbers, muster points, safety showers, eyewashes, shutoffs, spill boundaries, medical routes, and responder authority before work starts.
  • Maintain current hazard inventories, safety data, protocols, training, access, equipment, waste, exposure, and emergency contacts.
  • Ensure incident command can reach environmental health and safety, emergency services, medical, facilities, security, radiation or biosafety officers, and institutional leadership.

Procedure

  1. Prioritize life safety and incident command: raise the alarm, stop work, call trained responders, evacuate or shelter as instructed, account for people, and provide only trained first aid.
  2. Use eyewash, safety shower, emergency decontamination, or removal from exposure according to training and emergency guidance without delaying medical care.
  3. Establish hazard isolation by denying entry, controlling ignition or incompatible operations from a safe location, and communicating known and unknown hazards to responders.
  4. Do not reenter, touch equipment, clean a spill, move containers, or disable ventilation unless incident command confirms training, protective equipment, and conditions.
  5. Build an exposure roster with person, location, route, duration, symptoms, protective equipment, decontamination, medical destination, and follow-up while restricting health information.
  6. Conduct exposure and evidence preservation through scene custody, photographs when authorized, logs, alarms, samples, labels, waste, instrument state, access records, protocols, training, and witness accounts.
  7. Identify substances and processes from inventories and qualified analysis; treat unknown mixtures and missing labels as unknown hazards.
  8. Notify safety, environmental, occupational health, ethics, regulatory, insurer, worker, student, and community authorities according to applicable duties.
  9. Assess contamination, air or surface clearance, waste, ventilation, utilities, structural condition, equipment, neighboring rooms, research materials, animals, and environmental release.
  10. Permit cleanup, salvage, restart, and recovery authorization only through named competent authorities with documented controls and verification.
  11. Investigate causes and contributing conditions without blaming injured people; address design, maintenance, training, workload, supervision, labeling, procurement, and emergency readiness.
  12. Test corrective actions, update risk assessments and protocols, communicate lessons, monitor health and environment, and close only after required owners approve.

Failure plan

  • If the substance or energy source is unknown, expand isolation and wait for qualified hazardous-material response.
  • If a person is missing, injured, symptomatic, contaminated, or potentially exposed, notify incident command immediately and prioritize rescue by trained responders.
  • If a supervisor pressures staff to clean, restart, or conceal the event, preserve the instruction and escalate to independent safety and institutional authority.
  • If evidence collection conflicts with rescue, decontamination, containment, or medical care, life safety takes priority and responders document alterations later.
  • Never rely on odor, absence of symptoms, a silenced alarm, or informal wipe-down as clearance.

Worked example

A graduate researcher collapses after an unknown solvent release while ventilation alarms activate. Another worker's clothing may be contaminated, and the principal investigator wants the team to clean before emergency services arrive. Staff raise the alarm, evacuate, call responders, use trained decontamination steps, and report the missing chemical identity. Entry is controlled. Incident command coordinates medical care, hazardous-material identification, air monitoring, evidence custody, and notifications. The laboratory stays closed until competent safety and facilities owners document clearance, corrective controls, and restart authorization.
