Design an event feedback survey

Design a focused event survey that measures experience and intended outcomes without coercion or noise.

Procedure

  1. Define the decisions the survey must inform, target population, timing, channel, and owner.
  2. Map each question to an outcome, experience, barrier, or improvement decision.
  3. Use plain, neutral questions with balanced response options and an appropriate recall period.
  4. Separate satisfaction, learning, confidence, intent, access, safety, relevance, and open feedback.
  5. Keep identity and demographics optional, justified, protected, and separable from operational support.
  6. Provide accessible formats, language options, skip logic, progress, save, and assistance where needed.
  7. Pilot with representative participants and check completion time, interpretation, missingness, and harm.
  8. Publish purpose, consent, retention, analysis, reporting, and response-to-urgent-feedback rules.
